Diane Zuckerman, RPh
Diane Zuckerman, RPh, is a pharmacist and visionary entrepreneur who founded six (6) healthcare education and technology companies over the past 25 years. Her passion is embedded in reducing diagnostic errors by transforming the plethora of medical data into action, resulting in meaning and value. She focuses on the biopsychosoical model and patient-centered cognitive support. She always puts the patient first and promotes her mantra ‘doctors diagnose and treat patients, not diseases’. She creates solutions that provide high-value healthcare with a profound emphasis on wellness and early/correct diagnosis. Diane is a respected team leader, motivator and mentor with extensive knowledge of the healthcare clinical and business environment. She speaks on aligning quality care outcomes and economic incentives and the significance of preventive and behavioral responsibilities. She champions the dynamics of patient-clinician collaboration and the importance of communication and alliance of all stakeholders to insure a healthy ecosystem. The mainstay for all her strategic imperatives is entrenched in trust, integrity, generosity, and value. Diane was selected by Springboard Enterprises to be part of Springboard Trust. In 2009 she was chosen as one of 30 women in the US with the potential to be building a $Billion business in life sciences/healthcare. Diane was most recently chosen to be a member of the Dell Founders Club by Michael Dell who has committed $100M to 44 innovators & entrepreneurs with game-changing technologies. Diane is also on the BOD of UNWomen and continues her speaking and championing of healthcare, in particular women’s economic empowerment worldwide.

Leonard Fromer, MD
Dr. Leonard M. Fromer is Executive Medical Director, Group Practice Forum and Assistant Clinical Professor, Department of Family Medicine, University of California at Los Angeles. Dr. Fromer is a member of the board of TransforMED, LLC. He also acts as a consultant to TransforMED in the area of clinical integration: center of excellence quality improvement in the chronic diseases coupled with successful practice transformation to the patient centered medical home and the accountable care organization. As Executive Medical Director of the Group Practice Forum, Dr. Fromer leads a team engaged in national projects with group practices that deliver education, tools, and services to achieve success in their clinical integration efforts. Dr. Fromer lectures extensively on the topics of health-system reform, the patient-centered medical home and the accountable care organization. He has been featured on CBS News, ABC News, and in the New York Times. Leonard M. Fromer, MD, is a Fellow of the American Academy of Family Physicians, and a diplomat of both the American Board of Family Practice and the National Board of Medical Examiners. He has been in private practice in Santa Monica, California, with Prairie Medical Group for 28 years. Dr. Fromer has served on the group's Board of Directors and has been its Managing Director as well as its Chief Financial Officer. Dr. Fromer is a past president of the California Academy of Family Physicians and has served over ten years as a member and Chairman of the American Academy of Family Physicians Commission on Health Care Services. He has also been a member of the AAFP Commission on Quality and Practice. He has also served on the Physicians Capital Source Project Steering Committee for the American Medical Association, the Advisory Board of Directors for the American Medical Informatics Association and the Advisory Board for the World Foundation for Studies of Female Health.
Chief Financial Officer
Frederic Pothier
Mr. Pothier founded August France SAS (France), a luxury gourmet brand wholesaler. Previously, Mr. Pothier was CEO of Group Electrogeloz SA, a nationally recognized printing company based in Paris, France, where he introduced and fully integrated its digital workflow management system and its digital printing plant. Mr. Pothier was also the Executive Director of CAD Pass SA where he contributed to the transfer of French State confidential paper documents into raster- and vector-based digital document files. Mr. Pothier graduated from Ecole Superieur de Commerce (Paris, France) with a major in financial mathematics, and attended the California Institute of International Business (CIIB) in Berkeley, California with a major in Finance.

Peter Muldoon
Pete is also senior computer engineer for Bloomberg. Pete was previously with British Telecom, Merrill Lynch and Fidelity. Pete holds multiple patents in predictive algorithms, likelihood forecasts, and voice recognition. He is an expert in set theory, information theory and pattern recognition - used in voice recognition and computer-adided diagnosis. Mr. Muldoon was Master instructor Rhon Mizrachi's first black belt receiving his 1st Dan Black Belt in 1998, 2nd Dan Black Belt in 2001, and his 3rd Dan Black Belt in 2003 all from Haim Zut-President of Krav Maga International of Israel.
Medical Director
Leif Lundsford, MD
Dr Leif Lunsford grew up in northern California and completed his undergraduate studies at The University of California at Davis. He graduated from Boston University School of Medicine in 2002 and went on to complete his residency in Emergency Medicine at The University of Southern California in 2006. He continued to work in emergency departments in the Los Angles basin through 2012, however, he felt there was a better way to deliver more cost-effective, timely care to patients with non-life threatening conditions. In 2007 he co-founded what is now Exer–More than Urgent Care, an expanding group of urgent care clinics staffed by Emergency Medicine physicians. This unique enterprise brings the ER doctors out of the crowded hospital setting and places them in the community where patients can have quicker access to personalized care at a lower cost. Dr. Lunsford continues to engage in opportunities that improve patient care and physician job satisfaction.
